This is a discount plan and is NOT dental insurance. By enrolling, you attest that you do not have dental insurance and will not be enrolling in any. 

If at any point you do become active in a dental insurance plan, this membership plan becomes void IMMEDIATELY with NO REFUNDS. 

Membership fees must be paid in full (if annual) or first month paid (if monthly) with automatically-billed credit card stored on file and are non-refundable. 

Eden Lakes Dental reserves the right to cancel or discontinue this plan for any reason at the end of the Membership term.

Services not utilized may not be “rolled over” to the next year or transferred between family members, plan participants or any other individual. 

This Membership plan is effective on the date of this agreement and expires one calendar year after the date at the bottom of this agreement. 

Payment is due at time of service for any work not covered by this plan. 

This plan is accepted ONLY at the Eden Lakes Dental office.

This plan is not transferable. Family members cannot be substitutes for another family member.

This plan is non-refundable. No refunds are given if a patient chooses not to use their dental plan.

This plan includes services only. Products are not included in this discount. 

This discount does not apply to any past services or outstanding balances. 

Should there be dental treatment needed following any type of injury in which outside medical, care, disability, or workers comp type insurances are involved, this discount plan cannot be used.

Membership fees are subject to change on an annual basis.
